By submitting, I certify that the statements made herein are true and correct to the best of my knowledge and have been given voluntarily. Any statements that are found to be false (whether by willful knowledge or by accident) may be grounds from my dismissal from DAC’s volunteer program.
I understand that I will not be paid for my services as a volunteer and that submission of this application does not bind the Davis Arts Council, nor I, into any working relationship. Acceptance into Davis Arts Council’s Volunteer Program will be contingent upon successful interview and volunteer training.
